This MIORA Privacy Policy (“Privacy Policy”) describes the way "MIORA CO PC" and MIORA branded affiliates and subsidiaries, (collectively, “MIORA,” “we,” or “us”), collects, uses, stores, and discloses information about you when you interact with MIORA  through our website located at https://miora.lifetime.life, however accessed (the “MIORA Website”), or via our mobile applications (“App”) (collectively, the “Site”).”)

If a separate or different privacy policy applies to a specific App or Website operated by MIORA other than this Privacy Policy,  such App or Website shall indicate that by posting that separate specific privacy policy on that App or other Website (each, a “Specific Privacy Policy”). With respect to any other App or Website, that Specific Privacy Policy, rather than this Privacy Policy, will apply and describe the privacy practices on that other App or Website.

By interacting with MIORA through your use of our Site, you expressly and directly consent and agree to the practices set forth below.

1. Information We Collect

The information we collect from you varies, depending on the way you use our Site. MIORA may collect information from or about you in the ways described below, including directly from you, from your interactions with our Site, products and services, and from third parties.

Information You Provide Us

We collect information you provide directly to us. For example, we collect information directly from you when you schedule a consultation or communicate with us via third-party platforms.The types of information that we collect directly from you include your name, email address, phone number, state in which you reside, and any other information you choose to provide.

Other Information We Collect Automatically Through Your Technologies

We automatically collect information about your interactions with us or our Site, including:

  • Device, Usage, and Activity Information: We collect information about how you access our Site, including data about the device and network you use, such as your hardware model, operating system version, mobile network, IP address, browser type, unique device identifiers, and app version. We also collect information about the route by which you access our Site, access dates and times, and your activity on our Site, such as pages viewed, links clicked. We use Adobe Analytics to recognize you and link the devices you use when you visit our Site or services on your browser or mobile device. Adobe Analytics use cookies and other tracking technologies to help allow us to better understand how our users interact with our Site or services and to tailor our content to you. Depending on where you reside, you can opt out of our use of certain tracking through cookies and similar technologies on our website by clicking the Your Privacy Choices footer link. How We Use Adobe Analytics

Adobe Analytics enables us to collect and analyze certain types of your actions on our Site, such as your clicks on our Site. 4. DISCLOSURE OF INFORMATION

We disclose information about you as described in the  Analytics section above and in the following scenarios:

  • Vendors and Service Providers, including Life Time. We make personal information available to our vendors, service providers, contractors and consultants who perform services on our behalf, such as companies that assist us with web hosting, analytics, and marketing. As our managed services provider, we disclose personal information to Life Time who collects and processes information through our Site on our behalf to provide services to us.
  • Law Enforcement Authorities and Individuals Involved in Legal Proceedings. We disclose personal information in response to a request for information if we believe that disclosure is in accordance with, or required by, any applicable law, regulation, or legal process, including lawful requests by public authorities to meet national security or law enforcement requirements.
  • To Protect the Rights of MIORA and Others. We may disclose personal information if we believe that your actions are inconsistent with our user agreements or policies, if we believe that you have violated the law, or if we believe it is necessary to protect the rights, property, and safety of MIORA, our users, the public, or others.
  • Professional Advisors. We disclose personal information to our legal, financial, insurance, and other professional advisors where necessary to obtain advice or otherwise protect and manage our business interests.
  • Corporate Transactions. We disclose personal information in connection with, or during negotiations of, certain corporate transactions, including a merger, sale of company assets, financing, or acquisition of all or a portion of our business by another company.
  • Corporate Affiliates. Personal information may be disclosed between and among MIORA and our parents, affiliates, subsidiaries, and other companies under common control and ownership in accordance with applicable law.
  • With Your Consent or at Your Direction. We make personal information available to third parties when we have your consent or you intentionally direct us to do so.

We also disclose aggregated or de-identified information that cannot reasonably be used to identify you. We maintain and use this information only in a de-identified fashion and will not attempt to re-identify such information, except as permitted by law.

It is not shared.  Personal information is shared from Life Time to MIORA, but not MIORA to Life Time because that would be PHI.  Put another way, Life Time can share its member data, but MIORA can’t share patient information to Life Time.

5. Do not track mechanisms

The Site does not support Do Not Track (“DNT”) at this time, and so, we currently do not honor DNT signals. For more details, including how to turn on DNT, visit www.donottrack.us.

6. Changes to this privacy policy

MIORA reserves the right to update or modify this Privacy Policy at any time. If we make changes, we will post the revised Privacy Policy to our Site with a “Last Updated” date at the top of this Privacy Policy reflecting the date of the change. All changes are effective as of the time of posting. You should periodically check our Site to review any updated Privacy Policy. Without limitation to the effectiveness of the changes, your use of our Site following the posting of the revised Privacy Policy constitutes acceptance of those changes.
